Routine heart prevention
Does a daily fish-oil capsule prevent major heart events in generally healthy adults?
VITAL did not show a statistically significant reduction in its primary composite cardiovascular outcome. Some secondary findings differed, so the answer depends on the exact outcome.

The question behind the headline
25,871 older US adults; 1 g/day marine oil versus placebo, median 5.3 years.
What the finding does—and doesn’t—tell us
The major-event hazard ratio was 0.92 (95% CI 0.80–1.06). A secondary heart-attack signal does not make the primary result positive or establish benefit for everyone.

Three questions to take forward
- Was the study population similar to the person or situation I have in mind?
- Did it measure an outcome people feel or experience, or only a laboratory marker?
- How do formulation, usual care, absolute benefit and adverse effects change the interpretation?
